Fibromyalgia: Looking Beyond Pain and Inflammation
Fibromyalgia is often associated with widespread pain throughout the body and fatigue. However, for many people, the experience goes far deeper than that.
Although Fibromyalgia can affect both men and women, women are more commonly diagnosed and may experience additional hormonal influences throughout different stages of life.
Brain fog, digestive issues, hormonal imbalances, exhaustion, poor sleep, nervous system overwhelm and heightened sensitivity can all form part of the picture.
Many people living with Fibromyalgia are told their tests are “normal”, leaving them feeling frustrated, dismissed and unsure where to turn next.
While Fibromyalgia can be complex, taking a more holistic approach may help uncover some of the underlying factors contributing to symptoms and support the body more gently and sustainably.
Fibromyalgia Is About More Than Just Pain
Although chronic pain is one of the most recognised symptoms of Fibromyalgia, many women also experience:
• ongoing fatigue and exhaustion
• poor sleep quality
• brain fog and difficulty concentrating

• heightened stress sensitivity

Over time, this may leave the body feeling exhausted, overstimulated and unable to fully recover.
Stress does not mean symptoms are “all in your head”. Rather, the nervous system and body are deeply connected, and ongoing stress can place additional strain on the body’s ability to regulate and heal.

Hormones, Menstrual Cycles & Perimenopause
For many women, Fibromyalgia symptoms may also fluctuate alongside hormonal changes throughout different stages of life.
Some women notice their symptoms worsen:
• before or during their menstrual cycle
• during times of hormonal change
• throughout perimenopause
• during periods of increased stress or poor sleep
Painful periods, heavy bleeding, increased fatigue, headaches, mood changes and heightened sensitivity may sometimes occur alongside Fibromyalgia symptoms.
Hormones and the nervous system are closely connected, which means hormonal fluctuations can sometimes influence inflammation, sleep quality, stress resilience and pain sensitivity.
Perimenopause in particular can be a significant time for some women, as changing hormone levels may contribute to:
• increased fatigue
• poorer sleep quality
• brain fog
• anxiety or overwhelm
• muscle aches and joint discomfort
• worsening nervous system sensitivity
This is why taking a broader view of hormonal health, nervous system support, stress and recovery may form an important part of supporting women experiencing Fibromyalgia symptoms.
Fibromyalgia, Gut Health & The SIBO Connection
Many women living with Fibromyalgia also experience ongoing digestive symptoms such as:
• bloating
• reflux
• constipation
• loose bowels
• food sensitivities
• nausea
• abdominal discomfort
In some cases, these symptoms may be linked to underlying gut imbalances such as SIBO (Small Intestinal Bacterial Overgrowth).
SIBO occurs when bacteria begin to overgrow in the small intestine, which can contribute to:
• inflammation
• poor nutrient absorption
• fatigue
• nervous system stress
• brain fog
• immune activation
Emerging research has also explored possible links between Fibromyalgia and SIBO, with some studies finding a higher prevalence of SIBO in people experiencing chronic pain and fatigue conditions.
When digestion and gut health are not functioning well, it may place additional stress on the body and nervous system. This can potentially contribute to symptoms such as exhaustion, heightened sensitivity and widespread discomfort.
This is why, in clinical practice, looking at gut health can sometimes form an important part of supporting people with Fibromyalgia symptoms.
Functional testing such as microbiome testing or SIBO breath testing may help identify underlying digestive patterns that could be contributing to inflammation, fatigue and nervous system dysregulation.
Looking Beyond Standard Testing
Many people with Fibromyalgia are told their blood tests are “fine” despite still feeling unwell.
While standard pathology testing is important, it does not always provide the full picture.
Depending on individual symptoms and health history, additional functional testing may sometimes be considered, including:
• complete thyroid panels
• hormone testing
• iron and nutrient testing
• Hair Tissue Mineral Analysis (HTMA)
• microbiome testing
• SIBO breath testing
These tests may help identify patterns relating to stress, digestion, mineral balance, inflammation and fatigue.
A Gentle, Individualised Approach
There is no one-size-fits-all approach to Fibromyalgia.
Each woman's experience is different, which is why support should be personalised and sustainable rather than overwhelming.
In clinic, support may include focusing on:
• nervous system regulation

• mineral balance
• sleep and recovery
• reducing inflammation
• stress management strategies
